We are providers for the following insurance plans: Medicare, Blue Cross/Blue Shield HMO and PPO, United Healthcare, Cigna HMO and PPO, Pacific Care, Great West, Secure Horizons, Humana, PHCS, Mountain Medical Affiliates, Fortis, and other companies.
If we are not a participating provider for your network, we can access out of network benefits. We can access out of network for Sloans Lake. Out of network benefits may require a deductible, or a higher deductible, and/or higher out of pocket expenses per visit. These out of network benefits vary from plan to plan. We suggest you call your insurance company to determine your benefits under your plan.
If you chose not to utilize your insurance plan we charge the following: $100 for an initial evaluation, $95 for an hour, $80 for 45 minutes and $65 for 30 minutes. Fee is collected at time of treatment. We gladly accept, cash, checks or credit cards.
There are many out of network benefits that we can accept. This list includes: Aetna PPO and most major PPO's We will verify all insurance prior to the first visit to ascertain the benefits available and this will be communicated to the patient.
